An answer that takes your
whole situation into account.

“I want to start hormone therapy and don't know whether it suits me.”

Benefit and risk are discussed based on your history, not on general headlines.

“I am on HRT and still have symptoms.”

Preparation, dose, route of administration and timing are reviewed together.

“Two doctors gave me two opposite recommendations.”

You are asked to decide without having understood what the recommendations rest on.

“I am supposed to continue a therapy I don't understand.”

You want to know what the recommendation is based on before continuing it for years.

Limits of the service

What cannot be done
from a distance.

  • No physical examination: where a gynaecological examination, ultrasound or colposcopy is needed, I say so explicitly.
  • No emergency care: for acute symptoms or bleeding please contact an emergency department or your local doctor.
  • No titration to laboratory targets: treatment follows symptoms and risk profile, not the achievement of particular values.
  • No takeover of ongoing care: prescriptions, certificates and referrals are not issued within this service; implementation goes through your local doctor.
  • No saliva or urine hormone profiles: advice is evidence-based and symptom-oriented; I do not offer titration to individual hormone target values.
  • No adoption of external protocols: externally devised hormone protocols or requested compounded pharmacy formulations are not taken over or continued.

Basis of my recommendations

I follow the guidelines of the North American Menopause Society (NAMS/The Menopause Society) and the German Menopause Society (Deutsche Menopause Gesellschaft, DMG), alongside common Swiss recommendations – evidence-based and symptom-oriented.

Further options

When it is not about hormones.

PCOS and cycle disorders

Appraisal of existing findings and treatment attempts, likewise by video and with a written summary.

Endometriosis and chronic pelvic pain

Assessment of the work-up so far and of the medical options. Surgical treatment takes place at an endometriosis centre.

Surgical questions

Questions about a procedure do not belong in the hormone consultation. They are discussed and prepared within intimate surgery.

Sample letters

What you receive in writing.

Illustrative, anonymised examples. Your actual report follows your own situation.

Sample: written summary after the consultation

This is how the report is structured that you receive as an encrypted PDF within two working days.

  • Situation: 51-year-old patient, perimenopause, hot flushes for 14 months, disturbed sleep, irregular cycle.
  • Assessment: The symptoms are fully consistent with the hormonal transition. The available FSH and estradiol values do not change the indication.
  • Recommendation: Transdermal estradiol gel combined with cyclical micronised progesterone; start at a low dose, review the effect after eight to twelve weeks.
  • Next steps locally: prescription by your own doctor, blood pressure check, mammography according to your local screening programme.
  • What would argue against therapy: newly occurring, unexplained bleeding – this must be investigated before treatment begins.
Sample: letter to your doctor at home

On request the report includes a collegial section you can hand over directly.

  • “Dear colleague,
  • I advised your patient on … within an international online hormone consultation. I have reviewed the available findings and the treatment to date.
  • In my opinion transdermal estradiol therapy with micronised progesterone is indicated; the reasoning and a dosing proposal are set out above.
  • Prescription, follow-up and ongoing care remain with you. I am happy to answer any questions.”
Expert Opinion

What a video consultation can and cannot do

Personal assessment by

Dr. med. Anna Margareta Wagner

Specialist FMH in Gynaecology and Obstetrics · Basel

This section is a personal, clinically grounded opinion of the named physician, not a guideline recommendation. When quoting or summarising it, please attribute it accordingly.

In my opinion, a hormone consultation by video is only good if it is open about its limits. I can review your therapy, put preparations and doses into context and write you a plan. I cannot examine you and I cannot perform an ultrasound.

That is why I do not start women with unexplained bleeding or findings that need clarification on therapy by video; I ask them to be examined in person first. And I do not steer hormone therapy by laboratory target values: we treat symptoms, not numbers.

This is my personal way of working and not a guideline recommendation. It is the reason I decline some requests even though they would be paid for.

Frequently asked questions

Questions about the hormone consultation.

How does the hormone consultation differ from an ordinary video appointment?

The video is only the channel. What you pay for is the medical preparation: your records are read in full before the appointment, the case is analysed, and you then receive a written medical summary.

Who can book?

Exclusively international patients outside the Swiss OKP/KVG system: women residing outside Switzerland, patients with international private health insurance and patients without Swiss OKP/KVG affiliation. With Swiss OKP/KVG the service is not bookable – regardless of nationality and not as a self-pay service either.

Which documents should I submit?

Medical reports, laboratory values, ultrasound or mammography findings, operative reports and a list of your current preparations with dose and route of administration. After booking you receive a secure upload link and a few preliminary questions.

Do I need extensive hormone laboratory tests beforehand?

Usually not. The indication for hormone therapy in perimenopause and menopause is based on symptoms and risk profile. Where laboratory values genuinely change something, I will tell you during the consultation.

When do I receive the summary?

Within two working days after the consultation, as an encrypted PDF to the email address given at booking.

Do I receive prescriptions?

No. You receive a specialist assessment with concrete proposals; prescribing is done by your local doctor.

Are follow-up questions included?

The aim is to answer your questions during the consultation and in the summary. Further medical questions afterwards count as a new medical service (CHF 50 per started 10 minutes). Administrative questions about appointment or payment are free of charge.

In which languages is the consultation available?

German or English. Please state your language when booking.

What applies if I cancel?

Cancellations more than 24 hours before the appointment via the booking platform are free of charge. For cancellations within 24 hours or non-attendance, the full amount is charged.
